Before filling out this form, check the Central Systems page to determine if this is the appropriate way for you to obtain your suite of accounts on axe, redwood, and sorrel. If you need a Banner account contact the security administrator for your area.

This request will be processed within 24 working hours after it is received by the Operations Staff. At that time, activate your account at Account Activation. Your login id will be assigned to you when you finish the activation process. When an account is assigned it will automatically be assigned on axe, redwood, and sorrel.

It is also your responsibility to read the Appropriate Use Policy.

Note: All personnel accounts generated by submitting this form will consist of your initials and a 7001 series number and cannot be changed. (example: vjw7001)
