Risk Management

The Risk Management practices of Humboldt State University are specifically designed to identify and assess all types of risk to the campus, and to implement appropriate risk management techniques to protect the University and allow it to carry out its mission.  It is the goal of the RISK MANAGEMENT POLICY  (.doc) to aid in the identification and evaluation of risk, to facilitate the selection and application of the best risk management techniques, and to monitor the results. 

USE OF OFF-CAMPUS FACILITIES

EVENT PLANNING FORM (.pdf)  Use the Event Planning Form to assess and mitigate any potential risks that may be associated with your event. 

HOLD HARMLESS (INSURANCE) REQUEST FORM  (.xls)  When a department or club plans to use an off-campus facility for an activity, they should complete and submit a Hold Harmless Request Form.  Using the information provided, the appropriate agreement to the facility will be issued and a certificate of insurance from our self-insurance broker will be provided to the facility owner/manager.  Requests should be submitted at least 2 weeks prior to the event.

 

FIELD TRIP POLICY AND GUIDELINES

FIELD TRIP POLICY  (.pdf)  Humboldt State University requires that each college within the University develop a field trip policy appropriate to its field trip activities. A committee composed of members of the college and the university's risk manager will work in conjunction to develop these policies.  All policies will receive final approval from the dean of the college and the vice president.

VEHICLE USE AND ACCIDENT REPORTING

AUTO USE POLICY  (.pdf)  Humboldt State University follows the California State University's (CSU) policy regarding the use of vehicles on University Business.  Please review the Auto Use Policy prior to first use of University or private vehicle on University Business. 

VEHICLE ACCIDENT GUIDELINES (.doc)  Provides information on what to do If you are involved in a vehicle accident while on University business please refer to these guidelines for information.

MOTOR VEHICLE ACCIDENT REPORT (pdf.)  Must be completed by the vehicle driver involved in the accident and sent to this department within 24 hours of an accident.

STATE DRIVER ACCIDENT REVIEW (.pdf)  Must be completed by the driver's supervisor and submitted to this department within 24 hours of an accident.

 ACCIDENT (other than motor vehicle) AND LIABILITY CLAIM FORM

ACCIDENT REPORT (Non-Vehicle) (.pdf)  For all non-vehicle related accidents, complete and submit this form within 48 hours to this department.

GOVERNMENT CLAIMS PROGRAM INFORMATION AND CLAIM FORM (.pdf)  For real or personal property losses you believe were caused by the action or inaction of University.
