How Do I Report A Bias Incident?

Reporting incidents of bias or hate that you have experienced or witnessed is critical to our ability as a campus to respond to, provide education around, and work to prevent such incidents. It is the choice of the individual submitting the report whether or not to remain anonymous.

There are 3 ways to report a bias incident that you have either experienced or witnessed:

Option 1 (Online): To submit an online report, click here.
Note: This form is fillable.

Option 2 (Phone): To file a report via the BRT hotline. You can leave a voice message by calling 707-840-HSU1 (4781). The BRT Team will be checking messages daily and will contact you shortly. *For the quickest response, please file an online report.

Option 3 (In Person): To file a report in person, please fill out a paper report form, available at any of the following locations (see map of reporting locations below). You can also download and print the paper form, and then either mail to the address listed on the form or drop off at any of the locations below.

What Happens to Report?

Whether you file your report online, by phone, or in person, the Bias Response Coordinator who will contact you as soon as possible after receiving the report (if you have included contact information). To see a flowchart of what happens to your incident report, visit: "What Happens to My Report".
