At Humboldt State University the following information about a student can, by law, be released to the general public: name, addresses (on-campus residence hall addresses are not released to the public), email addresses, telephone numbers (on-campus residence hall telephone numbers are only released with prior permission of the resident), date and place of birth, major field of study, participation
in officially recognized activities and sports, photographs, dates of attendance, class level, enrollment status (full-time, part-time), degrees and awards received and the most recent previous educational agency or institution attended by the student.
No other student information is released to non-university personnel without your written permission. By completing this form, you will be requesting that information NOT be released to non-university personnel.
