Outstanding Student Awards: Students Who Make a Difference 2012-2013

 
DEADLINE TO SUBMIT:  Friday, February 22, 2013
Nominations are now being accepted for Humboldt’s annual Outstanding Student Awards. Faculty and staff are encouraged to nominate qualified candidates in the following categories.  Awards will be presented at our annual student recognition ceremony on Thursday, April 11, 2013, 3:00-5:00 pm in the Kate Buchanan Room. Formal invitations will be sent to all nominees and their nominators. 
 
PLEASE READ "NOMINATION PROCESS" and "AWARD CRITERIA" BELOW PRIOR TO STARTING THE ONLINE NOMINATION PROCESS.  All items listed should be submitted together. Your work will NOT be saved if you leave the form prior to submission. If you have difficulty attaching the nominee's photo or other materials, please email it to libolt@humboldt.edu. 

Read Nomination Process

All materials must be submitted by midnight on Friday, February 22, 2013.  Late nominations cannot be accepted.  If additional information regarding the nomination process is needed, contact the Office of the Vice President of Student Affairs at 826-3361.
 
Please submit complete nomination materials as follows:

  1. Fill in nominee's and nominator's Information in fields below;
  2. Attach nominee resume or letter :

-List those achievements, activities, awards, and other accomplishments attained while a student at HSU that relate to the nomination.

 -List other achievements, activities, awards and accomplishments attained while a student at HSU that are in addition to those mentioned in A above. (i.e. leadership positions, academic achievements, publications, committees, student government, club or organization involvement, community involvement, sports and recreation, etc.)

  1. Attach letter of support from the nominator(s) addressed to "Outstanding Student Awards Committee" describing the nominee’s achievements;
  2. Attach other appropriate materials for the committee’s review (i.e. posters, reports, art, etc. which can help the committee understand the student’s accomplishments);
  3. Attach jpeg photo of nominee for slide show which will be shown at the awards ceremony.

 

Read Award Criteria

The Awards Committee will consider the following applicable criteria when making their selections:

  • Nominee must be a student in good standing with at least a 2.5 cumulative GPA.   Award for Excellence in an Academic Discipline: 3.0 GPA
  • Volunteer versus paid positions: less emphasis will be placed on paid positions except when superior performance can be demonstrated
  • Accomplishments attained while the nominee was a student at HSU will be given priority; some achievements at prior institutions may be considered for transfer students.
  • Evidence of direct involvement
  • Level of commitment over a sustained period of time
  • Demonstrable and/or significant contributions
  • Inspiration to others
  • Innovation and creativity
  • Effectiveness
  • Enhanced and/or advanced the quality of student life
  • An individual may nominate only ONE student in each award category, except for the academic awards as follows:
    • Award for Excellence in an Academic Discipline:  An individual may nominate one student from each of the three academic Colleges.
    • Awards for Academic Excellence and Achievement:  An individual may nominate one student or team for each of the five categories.
  • A student, whose achievements clearly fit an award category other than that in which he or she was nominated, may be considered for a second category at the discretion of the Committee.

 Please provide a brief (one-line) description indicating the main reason you nominated this person (i.e. for commitment and dedication to the YES  Puentes program).   This description will be read at the awards ceremony.

 

Nominator Information
Outstanding Student Awards Nomination Categories (check one):

These awards will be presented to one student (or team) from each of the five categories for Academic Excellence and Achievement. A minimum cumulative 3.0 GPA is required for consideration.  Graduate students are NOT eligible for this award.
Nominees for an Award for Academic Excellence and Achievement will have excelled in the areas of research, publication, creative achievement and/or honors.

 

These awards will be presented to one student from each of the three academic Colleges who have excelled in an academic discipline (research, publications, creative achievements, honors, etc.) A minimum cumulative 3.0 GPA is required for consideration.  Graduate students are NOT eligible for this award.

 

 

Other Award Category Descriptions

Outstanding Contribution to a Campus Club, Program or Organization
Nominees in this category have provided energy, commitment, skills, or enthusiasm to a campus club or organization. The nominee may have served in a leadership position, put in quality time, or made an effort to implement an innovative program.
 
Outstanding Contribution to an Associated Students Program
Students nominated in this category have been actively and effectively involved in an Associated Students program or on the A.S. council.
 
Al Elpusan Award for Student Activism
This award has traditionally been conferred upon a student with a vision and commitment to make a positive change. This student will leave an enduring legacy at Humboldt State University after he or she departs. The student may have endeavored to build working coalitions and common ground, stepped beyond stereotypes, bridged communication gaps, brought together people from diverse backgrounds to work together, etc. This award is named in memory of the 1987‐88 HSU Associated Students President, Al Elpusan.
 
The Brian Lorensen Residence Life Staff Award
The Brian Lorensen Residence Life Staff Award celebrates a special element of leadership, the ability to inspire. The award is in honor of a student leader who gave of himself to everyone he met; who without even knowing, inspired and motivated; who found a way to see the positive in everyone and who relished in personal challenge and growth. This award goes to the Residence Life Staff who best exemplifies these qualities of inspiration, motivation, compassion and commitment to living and learning.
 
Award for Excellence in Intercollegiate Athletics or Sports Clubs
The student who receives this award has excelled as a scholar athlete. In addition to demonstrating athletic ability, this person has proven to be a respected leader on and off the field of play.
 
Award for Excellence in Community Service
Nominees in this category have made a clear commitment to serving the off-campus community.  They have made a significant contribution to an off-campus organization through program implementation, agency support, or in a leadership capacity.

 
Outstanding Students of the Year
Two students will be selected from within the top candidates of the above categories by the awards committee to be awarded Outstanding Students of the Year.
