RESOLUTION ON HUMBOLDT STATE UNIVERSITY
PROCEDURES FOR PROGRAM REINSTATEMENT (#09-03/04-EP)(#09-03/04-EP)
WHEREAS, Currently there is not a procedure for the reinstatement of suspended programs; and
WHEREAS, Occasions arise in which it is appropriate to reinstate suspended programs; therefore, be it
RESOLVED: That the Academic Senate of Humboldt State University recommend the adoption of the attached Humboldt State University Policy for Reinstatement of Suspended Programs.
HUMBOLDT STATE UNIVERSITY POLICY FOR REINSTATEMENT OF SUSPENDED DEGREE PROGRAMS

This document is a companion document to the Humboldt State University Procedures for Discontinuance of Academic Programs.
The faculty, chair of the department or individuals responsible for the oversight of the program may initiate reinstatement of a suspended degree program.
The initiating unit/individual shall provide a written justification for the reinstatement of the program addressing all the issues that were raised in the suspension document. The initiating unit/individual shall also notify all departments and the Library of the proposed reinstatement.
The proposal must be reviewed and approved by the appropriate departments, which shall provide comments and recommendations that shall be forwarded with the initiating document through the review process.
The proposal shall be submitted to the appropriate college curriculum committee and to the appropriate college dean for recommendation and comments. The college dean will provide a resource analysis. If the program to be reinstated is a University-wide program, the proposal shall be submitted directly to the University Curriculum Committee (UCC).
The initiating document incorporating the comments and recommendations from the college curriculum committee and college dean shall be forwarded to the University Curriculum Committee (UCC). In the case of graduate programs, the Graduate Council will review the proposal and recommendations and provide a recommendation to the UCC.
The UCC shall review the proposal and recommendations (including the Graduate Council's recommendation if appropriate) and make a recommendation to the Vice President for Academic Affairs and to the Academic Senate.
The Academic Senate will review the proposal and supporting documents and make a recommendation to the President. The Academic Senate will forward a copy of its recommendation to the Vice President for Academic Affairs.
After consultation with the Provost's Council, the Vice President for Academic Affairs will make a recommendation to the President.
The President will review the proposal and recommendations and make a final campus determination regarding the reinstatement of the degree program, and if favorable, inform the Chancellor's Office of the reinstatement of the program.
