Applying

The Educational Opportunity Program at Humboldt requires completion of the CSU application, as well as completion of the supplemental EOP application. Both applications are available at www.csumentor.edu. You can submit the EOP "Applicant Information Form" online, but only after you have submitted the CSU application. Also, EOP at Humboldt requires two "Recommendation Forms" to be completed, which must be downloaded and sent directly to the EOP office. You can find our address on the signature page of the EOP application and in the following application checklist.

If you do not have access to a printer, you can complete a paper version of the EOP application. You can request a paper application from your high school counselor or you can submit a request electronically and we will mail you an application.

EOP Application Checklist:

• Complete and submit the CSU application at www.csumentor.edu
• On the CSU application, check "yes" to the question, ”Do you wish to apply through the Educational Opportunity Program?”
• Complete and submit the EOP "Applicant Information" form
• Download and print two EOP recommendation forms
• Have the persons completing your recommendations send them directly to:

• Remember to separately complete the Free Application for Free Student Aid (FAFSA) by March 1, available at www.fafsa.ed.gov and the Cal Grant GPA Verification form for new applicants.
