General (Non-faculty) Confidentiality Statement Form
Any employee of the University or contractor or consultant (other than faculty members who must sign the Faculty Confidentiality Statement Form) who has access to personally identifiable or confidential data must have a signed general (non-faculty) confidentiality statement form on file with the office of Human Resources & Risk Management. Note, for purposes of this form, "non-faculty" is any one other than a faculty member with "faculty" broadly defined as tenure track, probationary, lecturers, counselors, coaches, emeriti, retired, graduate teaching assistants, and volunteers who teach class. In order to submit the form, follow these steps:
- Open the General (non-Faculty) Confidentiality Statement Form, read the statement, and either:
. Input your name and title, print the entire form, and sign and date it; OR
. Print the entire form, fill in (print) your name and title by hand, and sign and date it. - Make a copy of page 2 (the signature page) of the form and keep it and pages 1 and 3 as part of your records. Send the original (signed) copy of page 2 to your manager's office.
Your manager will sign page 2 and forward it to the Vice President for Administrative Affairs. The Vice President will sign page 2 and forward it to the Information Security Coordinator. The ISO will record in the University's security database that the form has been signed and send it to Human Resources & Risk Management for filing.
Filing of the form with HR&RM, is the first step toward being granted access. Access to University databases that contain personally identifiable or other confidential information also requires approval from the data custodian of that information. The data custodians are defined in University Management Letter 09-2003 [03-03], HSU Student Records Access Policy.
Two sources of information are provided to help you understand your responsibility to protect personally identifiable and other confidential information. While not mandatory, you should
- Read Information Use and Confidentiality Self-administered Training Materials. This document provides an outline of federal and state laws and policies that govern access to personally identifiable or confidential information.
- Do the FERPA Tutorial. This short, easy online tutorial provides a quick overview of your responsibilities under the federal Family Educational Rights and Privacy Act. Although this material is covered in the document above, the tutorial includes scenarios presented in a quiz format (non-graded, non-recorded) that can exercise or refresh you knowledge of complying with FERPA.
