standard University forms available as WordPerfect(.wpd), Word(.doc), or Excel(.xls).

Download notes:
Your web browser may already be configured to open up Word, WordPerfect
or Excel documents when you click on one of the files listed below.
If you click on a file and it displays garbage characters on the screen OR the
file does not open, then you will need to DOWNLOAD the file before opening it.
To Download
PC:
- Right-click on the file name | Left-click Save Target As | Select a place to
save where you will be able to find the document
- Open your application
Example: Open WordPerfect | Browse to where you saved your file and open the document
MAC: Hold the CTRL key on a Mac and click on the file entry. You will
then be prompted for a location to save the file.
