The Introduction should accomplish the following:

    1. Describe the purpose of the survey

    2. Explain how the survey recipients were selected

    3. Indicate that participation is voluntary

    4. Stress the importance of the recipient’s input

    5. Thank the recipient in advance for their time and effort

    6. Put the respondent at ease about the legitimacy of the survey

    7. Suggest how the subject matter is important to the respondent

    8. Indicate who will have access to the data and that the survey data is confidential

    9. Describe how the results will be used

    10. Emphasize the importance of complete and accurate answers

    11. Provide contact information for questions about the survey

    12. Estimate how long it will take for the respondent to complete the survey

    13. Clearly explain what to do to submit the completed survey

    14. Finally, provide instructions for completing the survey and describe any unusual conditions that will be
      encountered in completing the survey

    15. If using telephone surveys or face to face surveying, ask for permission to do the interview during the introduction.
