Travel Services

Before you make an appointment for a travel consult, please do the following:

Print the Travel Health History form and fill out completely
Go to the CDC website and print out all information on required/recommended vaccinations for your destination(s)
Make a copy of your vaccination records

After these three tasks have been completed, bring this paperwork to the Student Health Center for review by a nurse. If the information is complete, an appointment will be scheduled for your travel consult. An appointment will not be scheduled for you unless the requested paperwork has been completed.

The Travel Health History Form requires that you have Adobe Acrobat Reader installed. You can download and install it for free from the Adobe Website.

The Student Health Center is now furlough exempt and now open during most staff furlough days!

If you are going to travel this winter and plan to get your travel physical and vaccinations at the health center, please follow the instructions on the "Travel Services" page of the Health Center website. Five group travel clinics will be scheduled between November 3 & December 3, and ALL five travel clinics require that you drop off your travel packet BEFORE the clinic meets. Please plan ahead!

Students who need medical assistance when the Health Center is closed may call the Nurse Response Service at 1-866-724-5057.

Students: a hold will be placed on your ability to register for summer/fall unless the health center has copies of your immunization records. Please note that, even if you gave them to Admissions as part of your transcripts, you still need to file them with the health center.

The Student Health Center is now providing Family PACT reproductive health services. This state and federal program provides services for contraception, STD, women's annual exams, urinary tract infections, some labs and immunizations. Eligible students may sign up - for more information, contact the Student Health Center (826-3146).
