Please submit complete form to Sponsored Programs Foundation in SBS 427. Volunteers provide various types of services to the Sponsored Programs Foundation. This form Identifies the person as a Volunteer and makes them eligible for worker’s compensation insurance coverage if they are injured as a result of such volunteer services. Once completed it can be returned to SBS 427.
