Financial Aid Forms

1. Before you Log In for forms, please read below.
2. Read your AWARD GUIDE. It's all in there!
3. Review your TO DO list on your Student Center for requested forms or outstanding requirements.
4. The following forms require parent (or other) signatures. Complete the form, print and sign it and mail, fax, or email a scanned copy.  If you have any difficulty using the forms below, please see our Alternate Forms page.

   • Household Verification - Parent  (do not submit unless requested)
   • Request for Reanalysis - Special Circumstances (Dependent Students) - 39C1
   • Concurrent Enrollment - College of the Redwoods - 303/F
5. For all other forms, LOGIN to submit electronically. Your To Do list will be updated within 2 business days.  We will begin processing forms for the 2012-2013 Aid Year in the last week of March, 2012.
