Immunizations

Immunization Requirements for students entering Fall 2022:

The California State University (CSU) is committed to the protection of health and wellness of all students. To comply with this overarching goal, CSU campuses implement procedures to ensure that students are educated about and receive immunizations to prevent potentially serious and contagious diseases. Immunizations, Screening Requirements, and Recommendations are adopted from the California Department of Public Health (CDPH) IMMUNIZATION & SCREENING RECOMMENDATIONS FOR COLLEGE STUDENTS.  Please AVOID A HOLD on your REGISTRATION and submit proof of immunizations prior to deadline.

CSU students are REQUIRED to obtain the following Immunizations & Screenings:

      • COVID-19: Individuals must submit evidence of complete immunization (a single Johnson and Johnson vaccine or two doses of Moderna or Pfizer), plus a booster (if eligible).  For more information, see Campus Ready Testing and Vaccination page.
      • Measles, Mumps and Rubella (MMR): Two (2) doses with first dose on or after 1st birthday; OR positive titer (laboratory evidence of immunity to disease)
      • Hepatitis B (Hep B): Students age 18 and younger (CA Health & Safety Code, Sec. 120390.5)
      • Varicella (Chickenpox): Two (2) doses with first dose on or after 1st birthday; OR positive titer. History of contracting the disease does not suffice.
      • Tetanus, Diphtheria and Pertussis (Tdap): One (1) dose after 7 years of age.
      • Meningococcal conjugate (Serogroups A, C, Y, & W‐135): One (1) dose on or after age 16 for all students who are ages 21 years or younger.
      • Tuberculosis Screening/Risk Assessment (TB): Students will complete a questionnaire and any follow-up requirements.

Additional Requirements for those who are engaged in Fieldwork or Clinical Placements

All students enrolled in health care fields of study or teacher education programs are required to provide documentation regarding the completion of immunizations and health screenings as established by the state licensing agencies or clinical placement sites or fieldwork sites.  Students should contact the academic program for the specific required immunizations, health screenings (e.g., TB screening and physical examination) and any other fieldwork or clinical placement requirements.

We strongly encourage you to complete your immunizations with your primary care provider prior to coming to Cal Poly Humboldt.

Your primary care provider will be able to bill your insurance. However, if you are unable to do the the required immunizations before you start at Humboldt, the vaccines can be obtained at the Health Center for a fee. Fees for immunizations will be billed to your student account. We do not bill insurance. A hold will be placed on your ability to register for classes unless the health center has copies of your immunization records. Please note that even if you gave them to Admissions as part of your transcripts, you still need to file them with the health center.

How to Submit your immunization records

Upload a copy of your immunizations through the Secure Health Portal.

(Please note: The Health Center's Secure Health Portal is not to be confused with the Campus Portal - these are two separate systems. The Health Portal’s address is Health.Humboldt.edu)

Go to the Health Portal:

  1. Document Upload tab and select Immunization Records for the Document type.
  2. Upload a picture of your immunization records showing your name, date of birth, and clinical documentation.
      • If uploading a photo of your covid vaccination card, choose "Covid Vaccination Card" as the document type

If you have a hold on your student account for immunization records and you submit records meeting all requirements, your hold will be lifted in 24-48 hours of submission. If you are a new student, please submit these records before the to-do list item becomes a hold on your class registration for your second semester at Cal Poly Humboldt.

If you are submitting an EXEMPTION to a required immunization, complete the exemption form in the Health Portal (e.g., Exemption Form for Standard Immunizations and/or the COVID-19 Immunization). Please note that only MEDICAL exemptions are allowable for standard immunizations. In other words, RELIGIOUS exemptions are not applicable to standard immunizations. Either MEDICAL or RELIGIOUS exemptions are permitted in regard to the COVID-19 immunization. In the case of Medical Exemptions, ask your healthcare professional to complete the below form. Medical exemptions cannot be granted without this documentation.
