Some OLLI courses include field trips. Field trips have inherent risks. In order for you to participate in a OLLI course field trip, you must complete a Release of Liability form (PDF) and return it to the College of eLearning & Extended Education at least one week prior to the first field trip you attend.
A copy of this form will be sent to you with your registration receipt. It is also available on the HSU forms website (PDF). In some classes, the instructor will have copies of the form available. (PDFs require Adobe Reader or equivalent.)
