Hospitality Form

This PDF version is to expedite retrieval of signatures for ProCard reconciliations with multiple hospitality expenses. This form can be filled out and inserted into the ProCard reconcilation. All signatures can be collected at once through Adobe Sign.

For P2P PunchOut purchases, the hospitality form should be attached to your purchase order within P2P.

This form is for use with both ProCard reconciliations and for P2P PunchOut purchases where a hospitality form is required. 

NOTE: For off-campus venues, include justification for venue selection and consideration of on-campus alternatives in the event notes.
