INSTRUCTIONS:
- Form must be completed by the vehicle driver involved in the accident within 24 hours of the accident
- Supervisor must sign at the bottom as "Reviewing Officer" and must complete the STD 274 form:

- Supervisor will then submit both the STD 270 and STD 274 form to the Risk Management Office in SBS 311 within 48 hours of the accident
