SPF Pre-Appointment Request for Background Check

Submit completed form to SPF for processing.

SPF is committed to reasonably protecting all people who are employees, students, volunteers and guests. SPF will conduct background checks of all paid individuals and volunteers who voluntarily transfer or are hired into sensitive positions. For positions that require a Live Scan, please make an appointment with UPD and update SPF when the candidate has completed their appointment.
