Student Deaf and Hard of Hearing Services Acknowledgement

The University has changed its process beginning the Academic 2024-2025 year.  Please follow the directions below.

1. Students who wish to use the Deaf and Hard of Hearing Services accommodations must complete the Student Deaf and Hard of Hearing Services Acknowledgement. 
2. Return the completed form to the Campus Disability Resource Center by attaching the completed form to an email and sending the email to the Campus Disability Resource Center.
