Concerns and Action Guide for Faculty, Student Staff, Staff, & Administrators

Please navigate through the list below to identify the complaint type or issue that most accurately aligns with what you are experiencing. Each option will feature additional details, links to relevant policies and websites, as well as guidance on how to access the appropriate reporting form, if you choose to take action on addressing your complaint.

Academic dishonesty is willful and intentional fraud and deception to improve a grade or obtain course credit. More information about the policy and examples of violations can be found HERE

Basic steps and forms to report incidents are as follows:

  1. Faculty should notify the student, first, and meet to discuss the alleged infraction. At that meeting, the faculty will document the details using the Academic Dishonesty Referral Form and have the student sign the form.
  2. Faculty will then submit an Academic Dishonesty Reporting Form to the Office of Student Rights and Responsibilities and attach a pdf of the signed Referral Form. 

All Student Related Incident Reports (Motor Vehicle, Injuries, Property Damage, and/or Emergency Spills) are required to be submitted within 24 hours of the event. The Incident Report Form on this page is for student injuries, property damages, and reportable spills only. If the incident involves a motor vehicle, complete and return the Motor Vehicle Accident Report

Lab Specific Info:

  • As a general rule, a small (non-emergency) spill of a chemical that can be wiped up with a paper towel is not considered a reportable lab accident, but is considered general  housekeeping. Any large spills, extremely hazardous spills, spills requiring neutralization or spills involving other damage should be reported and must be addressed by faculty or staff (see Cal Poly Humboldt's Spill Response guidance document).
  • Getting chemical on oneself that requires either copious flushing for 15 mins or more(not simple handwashing), changing of clothes, neutralization, treatment or is toxic by absorption, corrosive or otherwise hazardous to health(carcinogenic, teratogenic, sensitizing etc...) MUST be reported on this form.
  • All spills of any chemical on a student or to determine if it is a housekeeping spill or a "reportable accident" must be reported to the faculty for assessment in all cases. If unsure whether it should be reported on the form, choosing to over-report so that tracking and trending can occur, to ensure EH&S can follow-up as needed is best. 

If this is employee related, complete the Supervisor's Report of Injury Form and/or the Motor Vehicle Accident Report & State Driver Accident Review if applicable. Also have the injured employee complete a DWC-1 Form (Employee Claim Form).

The Humboldt Ombuds service is open to all students, faculty, and staff. They offer a safe place to discuss problems or issues within the University and can help identify options for addressing specific concerns and resolving conflicts. Ombudspersons treat all inquiries as confidential (except as required by law or where there appears to be imminent risk of serious harm). Ombudspersons do not take a side in disputes. Overall, Ombudspersons are advocates for fairness and equity.

Office of Student Rights and Responsibilities

Campus Assistance, Response, and Engagement (CARE) provides assistance to students who are experiencing extenuating circumstances that are impacting their ability to focus on academics. Some examples are: houselessness, food insecurity, death of someone close, serious illness, etc. Limited, short-term resources can be provided by the University to help students get through challenging periods.

"Mandated Reporters" are identified (per E01083) based on their position at Cal Poly Humboldt. You will be notified by HR if this applies to you. What this means is that, in your professional capacity or within the scope of your employment, if you have knowledge of or observe a person under the age of 18 years whom you know or reasonably suspect has been the victim of child abuse or neglect, you must report the suspected incident, no matter where it occurred. (Penal Code §§ 11166(a)).

Grievances against Public Safety personnel, also known as police officers, must adhere to state mandated guidelines. These guidelines are made available by the University Police Department in the Lexipol Policy Manual. More information can also be found on the Commission on Peace Officer Standards and Training website.

Grievances against Cal Poly Humboldt auxiliary personnel follow separate procedures depending on the auxiliary organization. Auxiliary organizations include Cal Poly Humboldt Foundation, Sponsored Programs Foundation, and Associated Students.

All Cal Poly Humboldt employees (with the exception of a few medical and counseling staff) have a "duty to report" when they learn of any allegation of sexualized violence involving a member of our campus community.  

Sexualized violence includes, but is not limited to, sexual misconduct, sexual assault, sexual exploitation, dating violence, domestic violence, and stalking. More information about the violations can be found on the Title IX & DHR Prevention Office website.

The Humboldt Student Safety Escort Program provides Humboldt students and community members, who do not feel comfortable walking alone, with a uniformed escort to walk them between campus buildings, to their vehicle, residence halls, shuttle/bus stop, or other locations on the Main Campus. To request a safety escort, call 707-826-5555 or just dial 5555 from one of the campus courtesy phones. Blue lights are mounted near the outdoor courtesy phones for ease in locating them.
