Asset Management Forms
Use the forms below to report updates for non-IT assets. IT assets are limited to computers (desktops and laptops), tablets, and copiers/plotters and must be submitted through an ITS help desk ticket.
Choose the form that best matches the type of update you need:
Report a change to the physical location of a non-IT asset within your department, such as a building or room change.
Report the transfer of a non-IT asset between departments within the university. Asset Management will route the request for approval to the appropriate Delegation of Authority (DOA) on behalf of the requester.
Transfers to external organizations, including auxiliary organizations or other CSU campuses, must be submitted using the Disposal Form.
Request disposal of a non-IT asset that is no longer needed or no longer functional.
This includes transfers to external organizations, such as auxiliary organizations or other CSU campuses.
Property Loss Report (formerly HSU Form 71)
Incidents must be reported to University Police prior to submitting this form. Additional follow-up may be required.
