Telehealth Consent Form

If you are a client of the center, your therapist will send the Telehealth Consent Form to you through the SHC Health Portal for review and signature. A copy of the form can be found HERE. This allows you to be seen virtually via Zoom as needed (e.g. in the case of illness in which you prefer to keep your appointment). For those who prefer to be seen via Zoom on an ongoing basis, we recommend accessing services through a community provider.
