25Live is used to plan events both on and off-campus. This form will gather details about your event, provide information on any additional steps that need to be to taken to mitigate risk (i.e. Youth Protection Program, Insurance requests, etc.) and route to appropriate teams on campus for their approval (i.e. UPD, Risk, etc.).
Insurance Request Form
When a department or club plans to use an off-campus facility for an activity, they should complete and submit this form. A certificate of insurance from our self-insurance broker will be provided to the facility owner/manager. Requests should be submitted at least 2 weeks prior to the event.
