This financial hold is placed on your account if there is less than $199 past due.
Cashier's Financial Hold
This financial hold is placed on your account if there is more than $199 past due. This hold will prevent you from all services including registering for classes, ordering parking permits, applying for graduation, and preventing pending financial aid from disbursing.
When Holds are Removed.
Financial holds that have been resolved will be removed from the student account every odd-half-hour 24 hours a day (e.g. 9:30 am, 11:30 am, 1:30 pm, 3:30 pm, etc.). Please wait until this time after you have made a payment to see that the hold has been lifted.
For information on how to remove this hold, visit the Student Center Guide. The Enrollment Agreement Hold must be cleared before every semester. Once you have completed the requirements, the hold will be immediately lifted.

Your agreement to the terms and conditions contained herein are required for your registration at Cal Poly Humboldt.
Payment of Fees/Promise to Pay
I understand that when I register for any class or receive any service from Cal Poly Humboldt, I accept full responsibility to pay all tuition, fees, and other associated costs assessed as a result of my registration and/or receipt of services. I further understand and agree that my registration and acceptance of these terms constitutes a promissory note agreement (i.e., a financial obligation in the form of an educational loan as defined by the U.S. Bankruptcy Code at 11 U.S.C. ยง523(a)(8)) in which Cal Poly Humboldt is providing me educational services, deferring some or all of my payment obligation for those services, and I promise to pay for all assessed tuition, fees, and other associated costs by the published or assigned due date.
I understand that dropping courses or withdrawal from the University after the first day of classes may not result in a reduction of charges. I must adhere to the published deadlines and refund policy. I further understand that my failure to attend class or receive a bill does not absolve me of my financial responsibility as described above. I understand it is my responsibility to review my student center periodically for outstanding charges.
I understand that any questions regarding charges or payments on my student account can be directed to Student Financial Services. If you believe you do not owe all or part of an asserted unpaid obligation, you may contact Student Financial Services within 90 days of the charge due date. Additionally, Student Financial Services provides payment plans to assist students with financial responsibility.
Communication
I understand and acknowledge that my Cal Poly Humboldt email address is considered the official method of communication, including but not limited to billing notices, due dates, and collection notices. I am responsible for reading the emails I receive from Cal Poly Humboldt on a timely basis.
I understand that Cal Poly Humboldt uses electronic notification of outstanding debts and due dates as its official billing method, and therefore I am responsible for viewing and paying outstanding debts by their scheduled due date. I further understand that failure to review my account and due dates does not constitute a valid reason for not paying my debts on time.
I consent to receive my annual IRS 1098-T Tuition Statement electronically from Cal Poly Humboldt. I understand that if I do not consent to receive my Form 1098-T electronically, a paper copy will be provided. I understand that I can withdraw this consent or request a paper copy by contacting Student Financial Services at (707) 826-4407 before the end of the Fall semester.
Delinquent Account/Collection
I understand and agree that if payment in full is not received by the published or assigned due date, my account shall be considered delinquent. If my account becomes delinquent, Cal Poly Humboldt will place a financial hold on my student account, preventing registration for future classes, may cancel my enrollment, and assess late payment fees without further notice.
I understand and accept that if my account remains delinquent and I fail to make acceptable payment arrangements, Cal Poly Humboldt may refer my account to a collection agency and submit the debt to the State of California Franchise Tax Board for intercept of any monies owed to me by the State of California, as authorized under California Government Code Sections 12419.5, 12419.7, 12419.9, 12419.10, and 12419.11. My delinquent account may also be reported to one or more national credit bureaus and may result in a hold preventing future enrollment or issuance of a diploma.
I further understand that I may be responsible for all collection and legal costs incurred in the collection of unpaid charges. I understand that partial payments do not prevent my account from being considered delinquent unless I am enrolled in and complying with an approved payment plan. Returned or failed payments may result in additional fees and continued delinquency. Payments may be applied first to outstanding fees or prior balances.
Billing Errors
I understand that administrative, clerical or technical billing errors do not absolve me of my financial responsibility to pay the correct amount of tuition, fees and other associated financial obligations assessed as a result of my registration at Cal Poly Humboldt.
Privacy Rights & Responsibilities
I understand that Cal Poly Humboldt is bound by the Family Educational Rights and Privacy Act (FERPA) which prohibits Cal Poly Humboldt from releasing information from my education records without my permission, subject to certain exceptions under law. Therefore, I understand that if I want Cal Poly Humboldt to share information from my education records with someone else (e.g. family members, sponsors, third-party payer, etc.), I must provide permission by updating the Authorization to Release form online in my Student Center. I further understand that I may revoke my permission at any time by utilizing these same procedures.
Financial Aid Responsibilities
I understand that if I am a recipient of financial aid (grants, loans, scholarships, and/or employment), I must adhere to federal, state, and institutional policy, including the Satisfactory Academic Progress policy. I understand that any outside resources such as scholarships, stipends, service awards, contract payments, exemptions, or any other payment received from an outside agency may affect my financial aid award. I am responsible for notifying the Cal Poly Humboldt Financial Aid Office if I receive funds before or after receiving financial aid.
If some, or all, of my financial aid is revoked because I dropped or failed to attend class, I agree to repay all aid that was disbursed to my account and resulted in a credit balance that was refunded to me.
I understand that in certain situations, monetary prizes, awards, scholarships and grants awarded to me by Cal Poly Humboldt may be credited to my student account and applied toward any outstanding balance. I further understand that my receipt of a monetary prize, award, scholarship or grant may be considered a financial resource according to federal or state financial aid regulations, and may therefore reduce my eligibility for other federal and/or state financial aid (i.e., loans, grants, Federal Work Study) which, if already disbursed to my student account, must be reversed and returned to the aid source.
I will complete all applications, documents and requests for information accurately and promptly. Intentional misrepresentation on an application for federal or state financial aid is a violation of law and a criminal offense subject to penalties.

Authorization to Release

- Your student records that are maintained by California State Polytechnic University, Humboldt are protected under the Family Educational Rights and Privacy Act (FERPA). FERPA gives you the right to provide consent to disclosures of your education records to third parties, such as your parents, guardian, sponsor, etc.
- Upon completing the form below, you are authorizing the Offices of Student Financial Services, Housing and Residence Life and Financial Aid to share your records with your third-party designee(s). Granting access to one office does not grant access to the other offices.
A registration hold will be placed on your account unless the health center has copies of your immunization records. Please visit the immunization website to find all immunization requirements. Failure to submit the requested records will prevent you from all services including registering for classes, requesting transcripts, ordering parking permits, applying for graduation, and preventing pending financial aid from disbursing.
If you have not provided transcripts from another institution, you will have a hold placed on your student account. Failure to provide the requested documentation will prevent you from all services including registering for classes, requesting transcripts, ordering parking permits, applying for graduation, and preventing pending financial aid from disbursing.
