Entering the Respiratory Protection Program

Employees who are required to use respiratory protection must first complete the Respirator Protection Medical Questionnaire and Authorization for Service Form and submit both forms to:

Once medical clearance has been obtained, contact Environmental Health & Safety (EHS) to schedule the required respirator fit testing and training. Employees must complete the medical clearance and fit-testing process before using a respirator for work.
